We’ve added a new customizable Landing Page template to your account.
This is a great form if you want to collect a lot of information. By using custom and extra fields, you can create a great looking “survey” like one you would see at Survey Monkey.
Go to Settings > Landing Pages and click the Samples to add the newest page to your account. Here is a preview of the new page:
